When the Spiral Starts: What Your Body Is Actually Doing

The Alarm That Won't Turn Off

You noticed the thought an hour ago. You've examined it from every angle. You know, logically, that nothing new has happened. And still, the chest tightness, the shallow breath, the low-grade hum of dread sitting just behind your sternum.

This is the part nobody explains well. Not "why do I feel anxious", most people have a working theory for that, but why won't it stop. Why does the body keep firing the alarm after the reasoning mind has already called it a false positive?

The answer isn't that you're doing something wrong. It's that you're asking the wrong system to do the work.


A Fundamental Mechanism, and What Breaks It

Healthy brains do something quietly remarkable: they get bored of danger.

Present any stimulus repeatedly, a loud noise, a tense image, a concerning thought, and the brain's threat-detection system normally attenuates. The amygdala fires hard the first time, softer the second, softer still the third. This dampening process is called sensory habituation. It is a fundamental neural mechanism, one that protects the brain from sensory overload by reducing responses to persistent or repeated stimuli.

In anxious brains, this mechanism is compromised. While cognitive and behavioral inhibition in anxiety have been extensively studied, sensory habituation, how the brain attenuates responses to repetitive stimuli, has received far less attention; impaired habituation has been observed in psychiatric conditions and may reflect a failure of sensory gating that contributes to exaggerated vigilance and threat sensitivity.

So the alarm doesn't quiet. It re-fires on the same stimulus, as if encountering it for the first time.


What the Research Has Started to Untangle

A 2026 preprint from medRxiv went looking for the mechanism behind this. The researchers measured startle responses, across muscle, brainwave, and skin conductance signals, in participants with varying levels of trait anxiety, then used low-intensity focused ultrasound to probe whether directly modulating specific brain hubs could shift the pattern. The results showed that anxiety influences the rate of habituation, not the magnitude of initial startle responses, and does so primarily through somatic pathways. In other words: anxious people don't necessarily react bigger. They just stop dampening down.

That distinction matters. It means the problem isn't the size of the alarm. The brain's ability to filter sensory input is characteristic of healthy functioning, and disruption of this filtering leads to over-responsiveness and hypervigilance that may underlie pathological anxiety.

The paper specifically implicated the salience network, a set of brain regions including the anterior insula and anterior mid-cingulate cortex that flag what deserves attention. When these hubs don't brake properly, the body keeps being told: this is still urgent.

Earlier neuroimaging work pointed toward the same upstream failure. In a 2012 fMRI study published in Social Cognitive and Affective Neuroscience, participants with inhibited temperament, a precursor to anxiety disorders, showed a distinct pattern: their amygdalae failed to habituate to repeated stimuli, a failure the authors proposed reflects a social learning deficit underlying both behavioral avoidance and increased risk for anxiety disorder. The amygdala didn't quiet down. It held the alarm open.

A separate fMRI investigation of social anxiety disorder, published in PLOS ONE, found that a characterizing symptom is increased emotional reactivity toward potential social threat, and that despite activation of emotion-processing networks, little is understood about habituation in both the amygdala and the neocortical regulation areas that should be dampening it. The brakes exist. They just aren't being applied.


Why You Can't Think Your Way Out of the Body

Here is what the research makes concrete: the spiral is not primarily a thought problem. It's downstream of a sensory-gating failure that precedes conscious cognition.

By the time you're having the anxious thought, the body has already been running the alarm for a while. The thought is a product of the activation, not just a cause of it. And the prefrontal cortex, the reasoning, rationalizing part, gets progressively less access to the wheel the longer the alarm runs. A body locked in a threat state does not easily accept verbal reassurance. It doesn't read arguments.

Coordination of salience-network responses to negative emotional events relies on bottom-up signals from subcortical regions, including the amygdala, that are commonly implicated in abnormal emotional processing. Bottom-up. The signal goes upward from the body. Logic travels in the other direction.

This is why "just calm down," "it's not rational," and breathing exercises that feel forced rarely work, not because you're doing anything wrong, but because they are all top-down attempts to solve a bottom-up problem. The reset has to start in the body.


Four Things People Get Wrong About the Spiral

"I'm thinking myself into it." Partly. But the thought arrived because your sensory system had already primed the alarm. The thought and the body state are a loop, you didn't start it with the thought.

"If I could just understand why, it would stop." Understanding is useful for the long arc. It does not turn off an active alarm. A fire alarm does not stop because you correctly identify the source of the smoke.

"Distraction is avoidance." Not always. Introducing a competing sensory input, something physical, present, and neutral, is the mechanism by which bottom-up systems get redirected. This is not avoidance. It is sensory substitution.

"I have to feel it fully before I can let it go." Sometimes, yes. But some spirals are not insight problems, they are nervous-system loops that disengage more readily from a physical anchor than from prolonged internal focus.

"I should be over this by now." The research makes clear that the problem is a rate-of-dampening issue, not a character issue. The alarm re-fires faster than it should quiet down. That is a feature of how the system is tuned, not evidence of weakness.


Where the Body Needs to Start

If reasoning won't interrupt the spiral, what will?

The evidence points toward sensory input that is consistent, low-stakes, and non-threatening, input that gives the nervous system something genuinely present to process rather than another scan of the threat landscape. Rhythmic touch. Weighted or textured objects. A physical anchor that the hands can return to without having to think about it.

Is it normal for the spiral to start for no obvious reason? Yes. Because the mechanism is sensory, a failure to dampen repeated or sustained signals, the spiral can restart on the same stimulus the brain encountered earlier, even hours later, without any new trigger. This is not a sign the anxiety is worsening. It is how the gating failure expresses itself over time.
