The Attack Is Over. So Why Do You Feel Worse?
The panic has passed. Heart rate down. Breathing slower. You are, by most measures, safe.
And yet you feel hollow. Not calm, hollow. Like something scooped out the inside of your chest and left a faint hum in its place. The world looks slightly grey. You are neither here nor there. You might cry, or you might feel nothing at all, which is somehow worse.
Most explanations stop at "adrenaline crash" and move on. Drink water. Rest. It'll pass.
That framing is incomplete. What happens in the twenty to ninety minutes after a panic attack is a distinct neurobiological event, one with a mechanism and a specific window during which your nervous system is unusually receptive to physical comfort.
Two Systems. One Fires. One Doesn't.
The body runs two parallel stress highways. The first is fast: the sympathetic nervous system, which floods the body with adrenaline in seconds and produces the panic attack itself, racing heart, tunnel vision, impending catastrophe. The second is slower: the hypothalamic-pituitary-adrenal (HPA) axis, which releases cortisol over minutes to hours to sustain the stress response.
Here is the counterintuitive part. Graeff and Del-Ben's 2008 review shows the panic attack is driven primarily by midbrain structures operating through that fast sympathetic channel. The HPA axis, the slower cortisol system, responds very differently to panic than it does to ordinary stress.
An earlier paper by Graeff, Garcia-Leal, Del-Ben, and Guimarães (2005) documented that plasma cortisol levels rise sharply during anticipatory anxiety and generalised stress, but do not reliably rise during actual panic attacks, whether natural or laboratory-induced. The fast system fires. The slow system, curiously, holds back.
So the panic ends. Adrenaline clears within minutes. But the HPA axis, which has been running in a state of unusual suppression throughout, now has to recalibrate. That recalibration is not clean.
The Hollow Is a Rebound, Not a Rest
Prolonged or irregular HPA activation alters receptor density and feedback loops, leading to cortisol dysregulation that ripples into hippocampal structure, sleep, and emotional regulation.
After the acute panic signal drops away, the system doesn't return to baseline. Instead of remaining overactive, the HPA system can shift toward underactivity, lower cortisol output, producing flat affect, emotional numbness, brain fog, and loss of motivation.
That is the hollow. Not tiredness. Not drama. A measurable neurochemical state in which your emotional signalling system has gone quiet in the wrong direction.
These symptoms often follow sustained anxiety: chronic stress pushes cortisol up, but after prolonged activation the system can wear down enough that cortisol dips below normal. In the acute post-panic window, a compressed version of this same process plays out over an hour or two rather than weeks.
You are not being dramatic. Your nervous system is recalibrating a system that just ran hard in an unusual way.
When the Body Listens Again
The parasympathetic nervous system, the branch that governs rest and repair, begins reasserting itself as the sympathetic surge subsides. This is an active shift, not a passive one. Heart rate variability increases. Vagal tone rises. The body is pulling itself back toward regulation.
This window is when the nervous system is most physically receptive to co-regulation. Touch, warmth, and weight are processed through the C-tactile afferent system, a sensory pathway that feeds directly into the insula and prefrontal cortex, the regions responsible for body-awareness and emotional regulation. In panic-prone individuals, the HPA response may be poorly synchronized with autonomic activation, altering how long symptoms feel.

What People Misread
"I just need to sleep it off." Sleep after a panic attack is often elusive precisely because of the HPA recalibration still underway. Lying still while your emotional system flatlines is not rest. Active, gentle physical grounding before sleep tends to be more effective than waiting for calm in the dark.
"The emptiness means I'm depressed now." It doesn't. The post-panic hollow is time-limited and mechanistically different from a depressive episode, even though they share surface features, flatness and low motivation and slight unreality. Recognize the hollow for what it is: a rebound state, not a new condition descending.
"I should have been able to stop it." Graeff & Del-Ben's 2008 work maps how deeply subcortical panic responses are, driven by midbrain structures that operate faster than conscious thought. The attack was not a failure of willpower.
"Grounding techniques don't work for me after an attack." Cognitive grounding, name five things you see, count backwards, is harder in the post-panic window because prefrontal function is dampened. Somatic grounding, touch, temperature, physical weight, does not rely on the same cognitive resources, which is why it has a different point of entry.
"I should be over it by now." Neurochemical clearance takes time that the clock does not announce. Feeling off for an hour or two is biology, not self-indulgence.
